Registering with the Surgery
To register with the surgery you will need to complete a new patient registration form. Please complete the online version where possible as this will support us to register you. Alternatively there is a paper form available here (if needed, please ask as reception for this form).
Once you have done this, the team will review the registration request and process it. As part of it we will send you a second form to complete which asks for information that we will use for you medical care. Please note that without completing this second form, we will not be able to continue to have you as a patient. It is because this form provides important information that is critical to our team providing you safe and effective care.
When you register, it’s helpful to have your NHS number.
